My Grandmother is 88 years old and she is having trouble getting her passport.?

She was born in Minnesota in 1919, she was born at home and the only proof she has is her baptismal certificate. The Passport office sent her a phone number for Minnesota and it is disconnected. I tried searching on Ancestry.com and she shows up on the 1920 census. Can anyone help me get the right number for Minnesota Vital Statistics? Thank You.

How to obtain a birth certificate


1. Print and complete the Birth Certificate Application Form (see below).





2. If you are ordering certificates for more than one birth record, complete a separate application form for each record.





3. Complete the Birth Certificate Fee Worksheet to compute the charges for your order (see below). Applications without fees will not be processed.





4. Access to some birth records is restricted. See confidential birth records for more information. A birth certificate cannot be mailed to you unless you demonstrate tangible interest. You demonstrate tangible interest by checking the appropriate box on the application and by having your signature on the application notarized. Applications that are not notarized will not be processed.





5. Make your check or money order payable to Minnesota Department of Health. Checks returned for nonpayment will be charged a $30 fee according to Minnesota Statutes, section 604.113, subdivision 2, and civil penalties may be imposed for nonpayment.





6. Mail the completed form, birth certificate fee worksheet, and payment to:

Attention: Office of the State Registrar


P.O. Box 64882


St. Paul, Minnesota 55164-0882

Fax the completed form, birth certificate fee worksheet, credit card (Master Card, VISA, American Express and Discover Card) number and expiration date to 651-291-0101.

Reply:A similiar issue happened here in the Dallas area.. if I recall accurately, the family finally contacted the local representative/ congressperson office to undo the snag. Another source of information would be grandmother's social security file.


I understand the reasons for the more stringent requirements, but in the case here, the passport department was able to make an exception for valid cause.
Reply:A birth or death certificate or record may be obtained by mail or fax from the Minnesota Department of Health (MDH) or in person from a local registrar office.





You may also request a birth or death certificate or non-certified copy of a record via the Internet or fax by using US Certs (an independent business) to take your information and submit your application to the Office of the State Registrar.

Reply:I would strongly suggest you consider ordering the birth certificate through VitalChek. I've used them in the past and they are very fast and very efficient. If you know the county where your Grandmother was born, you could probably receive the certificate in 3 to 5 days for about $32 including FedEx shipping. Ordering through the state takes about 3 weeks minimum - even through VitalChek. Hope this helps.
Reply:Your grandmother has to have a birth certificate somewhere because birth certificates have been required in MN since 1907 regardless of where the baby was born.





Ancestry will only give you birth information for babies born in MN between 1935 and 2002.





Here's the website for the MN Dept of Health with instructions on how to obtain a certified birth certificate and their phone number. The passport office will not accept a non-certified copy.

It looks like you will have to fax or mail the forms and the wait time is 4-6 weeks.
